- There is no longer a need to complete a Covid Screening form prior to your appointment. However, if you are experiencing any Covid symptoms please postpone or cancel your appointment.
- All new patients are required to complete the Consultation Form before your appointment. If you are a returning patient and any of your personal or medical details have changed since your last appointment please fill in the Update Consultation Form.
- There is no waiting room so please remain in your car or outside and I will collect you at your appointment time.
- You do not need to wear a mask while at the clinic, but you are welcome to do so if you feel more comfortable. If you prefer I also wear a mask just let me know prior to your appointment it is not a problem.
Cancelation Policy
If you require to cancel or rearrange your appointment, we respectfully require 24 hour notice. This is so we can offer that appointment to others.
Cancelations or missed appointments without 24 hour notice will result in a part or full charge for the appointment.
We understand sometimes things will happen outside of your control however, we still require notice to prevent loss of time which may be allocated to others waiting.
